I choose to become a member of the Omega Community Labor Association, and understand that Omega Community Labor Association is a nonprofit, national labor organization that since 2006 has been providing Federally mandated and elective ERISA-based benefits and services exclusively to its member employees pursuant to a collective bargaining agreement which first recognizes my choice to make Omega Community Labor Association as my exclusive representative for collective bargaining regarding wages, hours, benefits and working conditions. I also understand that Omega Community Labor Association promotes such benefits at below private insurer market rates in order to help employers manage the costs and expenses to pay for and offer such benefits, and that the authority to run these member benefits resides in the ERISA laws of the United States, under the auspices and regulations of the US Department of Labor, ERISA ECE Section 3(40)(A).
